We can submit your claim to most Insurance companies but it is up to all patients to determine if our practice is IN or OUT of your Insurance network.
You can call your Insurance Company and speak to member services. They can look up the Physician by name and our location.
We are contracted with certain Insurance companies but just because we are contracted does not mean that you automatically have IN network benefits. Below is a list of Insurance companies we are allowed to submit to:

- Cigna (for HMO plans-card must have IL053, IL011 & IL300)

- First Health
- Health Link
- Humana Choice Care (we do not take Choice National POS)
- PHCS/Multiplan
- Tricare North
- Trustmark/Starmark
- UHC (we do not take CORE, NAVIGATE or COMPASS)

Any Self-pay Patients are expected to pay same day and a Self-pay discount will apply. We take major credit cards, personal checks and cash.
